Two new forms related to patient care—the Interdisciplinary Patient/Family Education Record and the Nursing Assessment Form—have been recently implemented at BWH.
The Interdisciplinary Patient/Family Education Record was designed to integrate the work of various disciplines involved in the patient’s education and to track the ongoing process of patient/family education according to JCAHO standards. Every patient admitted to the hospital must be engaged in a process of learning about health, changes that may result and any treatment that is prescribed. Every patient must have one of these forms initiated as part of their care at BWH.
The Nursing Assessment Form, derived from consolidating the Patient Health Data and Patient Health History Forms, was created to better assess the emotional support needs of patients and families, to meet new JCAHO requirements for nursing assessment, to include changes in pain management, falls prevention and DVT prevention standards and to facilitate the transition to an on-line nursing assessment form to be implemented in the near future. A nursing assessment form should be completed on all patients admitted to BWH within the time frames appropriate to the patient conditions. The time frames are documented in service/unit specific standards.
