The evolving relationship between BWH and Faulkner remains a pivotal part of both hospitals’ FY01 strategy. As clinical integration with Faulkner Hospital continues to grow, BWH physicians should follow the same policies at both sites, working collaboratively to provide optimum care for our patients. The physician who refers the patient for evaluation and/or admission should lead the responsibility in ensuring communication takes place between all members of the care team associated with the acute problems, past history, social history, and family history. Properly managing each case in this way will help to provide the patient with an efficient and complete evaluation.
The following instructions can be used as guidelines when admitting a patient to Faulkner Hospital.
EMERGENCY EVALUATIONS:
1. Please refer patients to Faulkner Emergency Department (617-983-7700) for evaluation. It is much easier to admit the patient to Faulkner if they have gone there directly, rather than BWH initially.
2. Call the Faulkner ED triage nurse (617-983-7701) to inform them about the patient and his or her condition. Be sure to provide the nurse with a follow up phone number and speak with the attending physician when clinically indicated.
3. Call in at the appropriate time interval, depending upon the clinical condition, to Faulkner ED to check on patient and assist with management and triage.
4. Faulkner ED attending will call receiving attending.
5. Primary MD should contact attending physician in the morning if patient is admitted overnight.
IF PATIENT IS TO BE ADMITTED DIRECTLY TO THE FLOOR:
1. If the patient is to be admitted to the hospitalist service, please call the hospitalist (732 - 6987 # 66171) to inform them about the patient. This is necessary to provide full clinical management information. Hospitalists are on site at Faulkner 7am- 5pm Monday –
Friday, and make rounds on weekends. Please call the hospitalist in morning if the patient is admitted overnight.
2. If the hospitalist is not in the hospital (5 p.m. – 7a.m.) at time of admission, the referring physician is responsible for the patient care. The hospitalist should then be contacted in the morning.
3. If the patient is to be admitted to a specialty physician, the referring MD must call the receiving physician.
4. Please call the house staff who will be caring for the patient to inform them about the patient and medical condition.
5. Please call Faulkner admitting (617 983 7152) to inform them of the direct admission.
