Tejal Gandhi, MD, will be leaving her role as executive director of Quality and Safety at BWH to become the director of Patient Safety for the Partners network in January. Gandhi, who created the BWH patient safety team in 2000, will continue her clinical and research activities as a member of the BWH Division of General Medicine.
Allen Kachalia, MD, will take over many of Gandhi’s responsibilities as he transitions to full-time medical director of Quality and Safety within the Center for Clinical Excellence. In this role, he will oversee the patient safety team, quality measurement and improvement team and CCE staff who support the Emergency Response committee and teams. He also will continue as an academic hospitalist in General Medicine.
Kachalia joined BWH in 2003 as an associate physician. He earned his MD from Washington University in St. Louis, his JD from the University of Pennsylvania, and his BS from the University of California at Los Angeles.
As Erin Graydon-Baker, MS, RRT, leaves her role in the Center for Clinical Excellence to become the Patient Safety manager for Partners HealthCare, Karen Fiumara, PharmD, has been named her successor as BWH Patient Safety manager.
Fiumara, who previously was the medication safety officer in Pharmacy Services, will oversee the BWH patient safety team and its hospital-wide initiatives while coordinating safety programs with other Partners institutions.
For the past three years, Fiumara was BWH’s medication safety officer. She also has led the BWH anticoagulation clinic for the last year. Prior to that, she served as the drug information specialist for BWH Pharmacy for three years. Fiumara has a doctorate of pharmacy experiential education from the Massachusetts College of Pharmacy and Health Sciences. She completed her residency at BWH.
