From now until the January 2004 JCAHO Survey, a weekly online "Ask Yourself" article series will be available at www.bwhpikenotes.org It is important for all staff to familiarize themselves with the content of these articles and to be prepared to answer related questions from a JCAHO Surveyor during the Survey.
ASK Yourself: "Why is there an increased emphasis on Patient Safety by the JCAHO?"
Since BWH’s last JCAHO Survey in 2001, JCAHO identified several new Patient Safety Standards. These standards were implemented in part as a result of the Institute of Medicine’s (IOM) 1999 report that indicated that up to 98,000 die each year from medical errors. BWH has long been a leader in conducting medical error research and implementing advanced prevention measures, including its computerized order entry system. To address this ongoing hospital priority, Tejal Gandhi, MD, heads-up BWH's Patient Safety Team, supported by Chief Medical Officer Anthony Whittemore, MD, and other clinical leaders at BWH and Partners-wide. The Team’s goal is to make BWH as safe as possible by reducing preventable medical errors and reinforcing a culture of safety through non-punitive reporting.
