On Nov. 2, a new web-based discharge module will replace the current BICS discharge information system, as well as all discharge dictations.
The new module, which will be accessed from the BWH inpatient pod monitor, facilitates multidisciplinary care, enabling all clinicians contributing to the discharge summary to see documentation generated by other clinicians. All disciplines will contribute to a single unified document.
The module also standardizes the discharge process, incorporating clinically pertinent information requested by care facilities as well as diagnosis-specific standardized discharge instructions and a streamlined medication reconciliation process. Documents generated by the module for referral facilities and providers, medical records and the patient all represent a significant improvement. In particular, patients will receive a new medication reconciliation summary and improved patient/family instructions, all in a comprehensive, easy-to-read format.
Training to introduce clinicians to the new module is underway. Clinicians are encouraged to complete the discharge module computer-based training in HealthStream prior to Nov. 2 in order to ensure that they and their patients receive the full benefits of the new discharge module.
