Do you know how patient learning needs are met?
The assessment of patients’ learning needs usually takes place in the primary care practices, the Pre-Admitting Test Center (PATC) or the Emergency Department. Each of these sites has systems in place for documenting the assessment of learning needs. Specific questions about learning needs, interests and preferences are part of the BWH Patient Health History. If hospitalized, ongoing patient assessments are made throughout the patient’s stay and are documented on patient care flow sheets, progress notes and discharge instructions. Information based upon their capability and readiness to learn and the financial implications of care are discussed with patients and their families prior to an elective hospitalization. The plan of care addresses patient education in medication use, medical equipment use, drug/food interactions, rehabilitation techniques, access to community services and how to obtain further treatment (if necessary) upon discharge. BWH publicizes patient responsibilities and rights in its patient guide, which is available to every hospitalized patient. Various services have also created videos to help patients and their families understand how they will be involved in care. Patient and family education is also provided when there is a change in health status, new treatment for an existing health problem or a need related to diagnostic or therapeutic procedures. Such education is generally provided: • At any point of entry, based on the assessment of needs. Print and on-line materials are available in different languages to address and/or reinforce prevalent issues. • Prior to admission by mail from the physician practice sites and during visits to the Pre-Admitting Test Center for patients who are scheduled for elective diagnostic and surgical treatment. • Prior to discharge from a hospitalization or at the end of any visit to the hospital.
