BWH announced the opening of a new primary care practice - the hospital's second patient-centered medical home - in Norwood last month.
"Primary care will be delivered by a highly integrated team that includes a primary care physician, physician assistant, social worker, medical assistant and a licensed practice nurse," said Kate Donovan, practice manager of Brigham and Women's Primary Care Associates at Norwood. "By providing needed primary and preventive medical care, we are making a major contribution to the health of our community."
When fully staffed, the new practice, which consists of 12 primary care exam rooms and three consultation rooms, will house six physicians and their care teams. The practice uses a team-based model of care, with the goal of providing comprehensive and coordinated care, expanded access and a positive experience for patients, their families and staff.
The Norwood practice will also offer innovative models of patient-centered primary care delivery, including group visits and educational sessions.
"We are planning on starting group visits later this year for patients with a shared diagnosis who are interested in this kind of appointment," said Donna Michelson, MD, medical director of the Norwood practice. "The group format maximizes education, shares best practices and enables patients to receive support and encouragement from each other."
