Effective May 1, 2002, BWH will begin to strictly enforce its documentation policy for clinical lab test orders on outpatients. The performance of these services without a complete laboratory requisition is not reimbursable. From May 1 to June 30, temporary staff will be located at the phlebotomy sites to screen incoming laboratory requisitions and facilitate documentation. Forms that are not fully documented will be returned immediately to the ordering provider for completion. Laboratory services will not be provided until the requisition form is complete.
