Reporting System Simplified
During Patient Safety Week in March, BWH staff filed 38.7 percent more safety reports via the Safety Reporting System than the weekly average since the beginning of this year.
In the last month, Patient Safety rolled out a new, improved version of the Safety Reporting System Form. “To simplify reporting, we removed 13 optional questions in the safety reporting system,” said Pilar Conde, patient safety analyst in the Center for Clinical Excellence.
For example, the number of questions involving medication/IV incidents was reduced from 32 to 19, and general incident types were reduced from 26 to 13 questions. These changes reduced data entry from five screens to four.
Also, the Safety Reporting guidelines have been updated. View the guidelines online at www.BWHPikeNotes.org From the home page, select Compliance Corner from the Staff Resources drop down menu, then select Risk Management on the left navigation bar and look for the Adverse Event Reporting Policy.
To file a safety report, go to the Start menu on your computer and select Partners Applications to get to safety reporting. Patient Safety wants to hear from you whether this change minimizes the time it takes for you to file a safety report. Contact pconde@partners.org
